The Network of Jewish Human Service Agencies (NJHSA) invites the participation of NJHSA US member agencies in good standing to join the expansion of its National Network Program, which is funded by a grant from the Jewish Federations of North America and its Center on Aging and Trauma to provide for Innovations in Person-Centered, Trauma-informed Supportive Services for Older Adults with Histories of Trauma, utilizing technology to address social isolation. The expansion of this program includes utilizing technology not just to assist older adults, but also to provide supports to their family caregivers.
NJHSA proposes to establish a partnership with Uniper Care to bring the Uniper Care technology to a maximum of 4 new participating NJHSA member agencies. These agencies will join the existing National Network cohort of 10 agencies that are currently providing the Uniper Care technology to Holocaust survivors and other older adults with histories of trauma. The new agencies will not only serve older adults with histories of trauma who struggle with social isolation, but also their family caregivers. Target older adult populations can include Holocaust Survivors; older adults who are Refugees, Victims of Domestic Violence or Veterans; older adults who have experienced racial, economic or gender discrimination; or those with histories of trauma resulting in the need for ongoing mental health and other supportive services. Vulnerable, at-risk and frail older adults with particular struggles associated with the impact of COVID-19 may also be considered for this grant.
The funding to be made available through this RFP will be a one-year grant (January 1 – December 31, 2022) to support core costs associated with implementing the Uniper Care technology. Agencies are asked to provide required staffing as needed. Those staffing costs may represent required match contributions. Participating agencies will be required to document a match of at least 25% to support related costs (administrative costs, social work case management staffing, program expenses, etc.) associated with implementing the technology and also programming, which can be shared via the Uniper platform, to support older adults with histories of trauma and family caregivers. Agencies will also be required to provide details on plans to sustain the service beyond the grant period.
The total funding available to NJHSA for this grant will not exceed $600,000, beginning in January 2022. The funds will be divided among a maximum of 10 continuing and 4 new participating NJHSA agencies, with the goal for each new participating agency to serve a maximum of 81 older adults with histories of trauma who struggle with social isolation and 13 family caregivers of these older adults. Agencies may choose to allocate participating clients between the Uniper TV version and the Uniper web-based version (for clients who prefer access via computers or tablets as opposed to television sets). It is assumed that family caregivers will use the web-used version. Agencies may also estimate the number of clients (up to a maximum of 20% of participating TV clients) who will need assistance with securing access to internet connectivity (required for both options). Uniper can make arrangements for those clients requiring such access at a subsidized rate.
New agencies will also be required as part of their agency programming on the Uniper platform to provide caregiver support content/programming. Examples are support groups, workshops, speakers, etc. Agencies are encouraged to make available their caregiving programming to other agencies in the cohort.
